Name Changes
Please consult our Passport Wizard to determine which form to fill out. If you are submitting a DS-11, you must make an appointment and apply in person. If you are submitting a DS-82 or DS-5504, please use the courier service to mail in your materials.
Documents needed to exchange your passport due to a name change:
- Passport application must be filled out using the Passport Wizard. You must provide a Social Security number, if you have been issued one. Once completed, please print and sign the application form.
- Your most recent passport
- Evidence of name change such as an original/certified marriage certificate, divorce decree, or court order. We will return these to you at your appointment or with your new passport if applying by courier.
- One passport photograph (sized 2″ X 2″ or 5 cm X 5 cm with a white background taken within the last six months/no eye glasses/no uniforms/no digital manipulation or retouching. Face coverings (i.e. bandages, masks) that block portions of the face are not allowed. We need to see your full face to establish your identity. Please see the travel.state.gov passport photo webpage for more information. U.S. passport and Korean passport photo sizes are different.)
- No additional fees if renewing within one year of issuance (A passport applicant who wants to have his/her passport changed to reflect a name change may receive a free replacement passport if the requested change is made within one year from the date of the original passport issuance.)
- Fee (if applying more than one year from issuance). The fee for minors is $135. The fee for an adult name change due to marriage, divorce, or court order is $130. The fee for an adult name change for any other reason is $165. Please see our fee payment page for acceptable methods of payment. A passport applicant who wants to have his/her passport changed to reflect a name change may receive a free replacement passport if the requested change is made within one year from the date of the original passport issuance. Please submit an original/certified marriage certificate, divorce decree, or court order.

Correction of Printing Errors
A passport applicant who wants to have biographical data corrected in his/her passport due to Department error may receive a free replacement passport with corrected data at any time.
Documents needed to exchange your passport for correction of printing errors:
- Passport application must be filled out using the Passport Wizard. You must provide a Social Security number, if you have been issued one. Once completed, please print and sign the application form.
- No additional fees
- Your passport with the incorrect biographical data
- If there is a printing error in the descriptive data, please submit evidence showing the correct information such as an original or certified birth certificate or a marriage certificate.
- One passport photograph (Sized 2″ X 2″ (or 5 cm X 5 cm) with a white background taken within the last six months/no eye glasses/no uniforms/no digital manipulation or retouching. Face coverings (i.e. bandages, masks) that block portions of the face are not allowed. We need to see your full face to establish your identity. Passport Cards
The U.S. Passport Card can be used to enter the United States from Canada, Mexico, the Caribbean, and Bermuda at land border crossings or sea ports-of-entry and is more convenient and less expensive than a passport book. The passport card cannot be used for international travel by air.
All U.S. passport card applications submitted at the U.S. Embassies and Consulates are sent to the United States for processing. If you are applying for a passport card along with a passport book, please note that they are processed separately. The processing of a passport card can take an additional month or two and can be mailed to you separately.
